The purpose of this form is to request new snippets or amendments to existing snippets on ERMS request forms.

A snippet is key instructions and/or important information for the referrer to check before completing a request form.

To keep snippets user friendly and aligned with the relevant request page or pathway consider the snippet guidelines over the page before completing the table below.

Snippet Guidelines:

Content:
- Include links for relevant CHP, HHP and AHW pathway or request page. Where AHW has no relevant pathway use the homepage link.
- Provide the following where relevant:
- Essential items under a “Requests must include” subheading. Items listed may include key clinical items which the request will be declined if missing, and that may not be clinically obvious or part of the standard assessment for that condition. For example, BMI for surgery, creatinine, scoring tools, growth charts
- Key administration information e.g., if referrers sometimes use an incorrect form or provider
Layout:
- Snippets should be short and concise. Where possible limit to 50 words or less.
- Use bullet points.
Approval Process:
- HealthPathways program coordinators manage snippet request changes and work with ERMS to coordinate these to ensure alignment with HealthPathways. The coordinators work with a Clinical Editor and subject matter experts as required.
- Send the information in the ‘snippet change request’ box (above, could cut and paste box or attach if preferred) to ERMS@pegasus.org.nz
- All requests must be approved by ERMS. ERMS confirms request conforms with snippet guidelines. If ERMS receive snippet change requests directly from people other than the HealthPathways program coordinators or ERMS Clinical Leader, direct request to the HealthPathways program coordinators for approval.
- Any snippets that don’t conform to this standard template require approval from the ERMS Clinical Lead.
